Tildrakizumab-asmn (IlumyaTM), risankizumab-rzaa (Skyrizi ®) and mirikizumab-mrkz (OmvohTM) are humanized IgG1/k, IgG1, and IgG4 monoclonal antibodies respectively, that selectively and specifically bind to the p19 subunit of interleukin-23(IL-23) and inhibit its interaction with the IL-23 receptor. IL-23 is a naturally occurring cytokine that is involved in inflammatory and immune responses. Tildrakizumab-asmn (IlumyaTM), risankizumab-rzaa (Skyrizi ®) and mirikizumab-mrkz (OmvohTM) inhibit the release of proinflammatory cytokines and chemokines.

Policy Position

Tildrakizumab (Ilumya) Subcutaneous (SC)

Tildrakizumab (Ilumya) may be considered medically necessary when an individual meets the criteria for the following indication:

  • Plaque Psoriasis (PsO)
    • The individual is 18 years of age or older with moderate or severe PsO; and
    • Treatment with phototherapy (e.g. PUVA, UVB) was ineffective, not tolerated or contraindicated. If member is not a candidate for phototherapy treatment (e.g. phototherapy is contraindicated due to history of lupus erythematosus, porphyria, or xeroderma pigmentosum), treatment with systemic therapy (e.g. methotrexate, cyclosporine) must have been ineffective or not tolerated, or all systemic therapies are contraindicated; or
    • Treatment with systemic therapy (e.g. methotrexate, cyclosporine) was ineffective or not tolerated, or all systemic therapies are contraindicated; and
    • Initial authorization will be for a period of 12 months.

Reauthorization Criteria

Continuation of therapy with tildrakizumab (Ilumya) may be considered medically necessary when the following criteria are met:

  • Prescriber attestation that individual has positive clinical response or demonstrated disease stability with use of therapy; and
  • Reauthorization will be for a period of 12 months.

The use of tildrakizumab (Ilumya) not meeting the criteria as indicated in this policy is considered not medically necessary.

Risankizumab-rzaa (Skyrizi) Intravenous (IV)

Risankizumab-rzaa (Skyrizi) IV may be considered medically necessary for induction therapy when an individual meets the criteria for ONE (1) or more of the following indications:

Crohn’s Disease (CD)

  • The individual is 18 years of age or older with moderate to severe CD; and
  • Initial authorization will be for a period of six (6) months; or

Ulcerative Colitis (UC)

  • The individual is 18 years of age or older with moderate to severe UC; and
  • Initial authorization will be for a period of six (6) months.

The use of risankizumab-rzaa (Skyrizi IV) not meeting the criteria as indicated in this policy is considered not medically necessary.

Mirikizumab-mrkz (Omvoh) Intravenous (IV)

Mirikizumab-mrkz (Omvoh) may be considered medically necessary for induction therapy when an individual meets the criteria for the following indication:

Ulcerative Colitis (UC)

  • The individual is 18 years of age or older with moderate to severe UC; and
  • Initial authorization will be for a period of six (6) months.

This therapy requires an IV induction, followed by a maintenance SC regimen that is managed under the Pharmacy Benefit. Additional coverage criteria apply in accordance with each individual's benefit design and corresponding utilization management and formulary provisions. Please review all requirements prior to initiation of an induction dose.

The use of Mirikizumab-mrkz (Omvoh) IV not meeting the criteria as indicated in this policy is considered not medically necessary.

NOTE: In addition to the above criteria, product specific dosage and/or frequency limits may apply in accordance with the U.S. Food and Drug Administration (FDA)-approved product prescribing information, national compendia, Centers for Medicare and Medicaid Services (CMS) and other peer reviewed resources or evidence-based guidelines. Highmark may deny, in full or in part, reimbursement for utilization that does not fall within the applicable dosage and/or frequency limits.

Place of Service: Outpatient

The administration of tildrakizumab (Ilumya), risankizumab-rzaa (Skyrizi) and mirikizumab-mrkz (Omvoh) are typically an outpatient procedure which is only eligible for coverage as an inpatient procedure in special circumstances, including, but not limited to, the presence of a co-morbid condition that would require monitoring in a more controlled environment such as the inpatient setting.
