Elivaldogene autotemcel (Skysona®) is a one-time gene therapy indicated to slow the progression of neurologic dysfunction in pediatric males with early, active cerebral adrenoleukodystrophy (CALD) and for whom a human leukocyte antigen (HLA)-matched sibling hematopoietic stem cell (HSC) donor is not available.

Eli-celgene therapy adds functional copies of the human adenosine triphosphate binding cassette, sub family D, member 1 (ABCD1) gene into patients’ HSCs ex-vivo through transduction of autologous CD34+ cells with Lenti-D LVV. The addition of a functional gene allows patients to produce adrenoleukodystrophy protein (ALDP), which is thought to allow for the breakdown of very long chain fatty acids (VLCFAs) that otherwise accumulate to toxic levels in the brain causing inflammation and demyelination.

Policy Position

Elivaldogene autotemcel (Skysona) may be considered medically necessary when ALL the following criteria are met:

  • Individual is a male ages 4 to 17 years at time of treatment decision; and
  • Individual is diagnosed with active cerebral adrenoleukodystrophy (CALD) defined by inclusion of ALL of the following:
    • Neurologic function score (NFS) less than or equal to 1; and
    • Elevated very long chain fatty acids (VLCFA) values; or
    • Hemizygous mutation in ABCD1; and
    • Active CNS disease established by brain magnetic resonance imaging (MRI) demonstrating BOTH of the following:
      • Loes score between 0.5 and 9; and
      • Gadolinium enhancement of demyelinating lesions; and
  • Prescribed by or in consultation with a neurologist or specialist with experience and expertise in the treatment of CALD; and
  • Individual does NOT have any of the following:
    • Prior allo-HSCT or gene therapy; and
    • Antibodies to the lentiviral vector; and
    • Severe hepatic impairment.

Note: The safety and effectiveness of repeat administration of elivaldogene autotemcel (Skysona) has not been evaluated. Therefore, coverage will be limited to once per lifetime.

The use of elivaldogene autotemcel (Skysona) not meeting the criteria as indicated in this policy is considered not medically necessary.

NOTE: In addition to the above criteria, product specific dosage and/or frequency limits may apply in accordance with the U.S. Food and Drug Administration (FDA)-approved product prescribing information, national compendia, Centers for Medicare and Medicaid Services (CMS) and other peer reviewed resources or evidence-based guidelines. Highmark may deny, in full or in part, reimbursement for utilization that does not fall within the applicable dosage and/or frequency limits.

Place of Service: Inpatient/Outpatient

Elivaldogene autotemcel (Skysona)  is typically an outpatient procedure which is only eligible for coverage as an inpatient procedure in special circumstances, including, but not limited to, the presence of a co-morbid condition that would require monitoring in a more controlled environment such as the inpatient setting.
