Ublituximab-xiiy (Briumvi™) is a cytolytic antibody that binds to CD20 on the surface of B cells and depletes them from circulation in the blood. Ublituximab-xiiy (Briumvi) is approved by the FDA for treatment of relapsing forms of multiple sclerosis (MS) in adults, which include clinically isolated syndrome, relapsed-remitting MS and active secondary progressive MS.

Policy Position

Ublituximab-xiiy (Briumvi)  may be considered medically necessary for the treatment of relapsing forms of MS, to include clinically isolated syndrome, relapsing-remitting disease, and secondary progressive disease, when ALL of the following are met:

  • Adult individuals 18 years of age or older; and
  • A diagnosis of relapsing forms of MS as defined by an MRI of the brain showing abnormalities consistent with MS; and
  • Prescribed by, or in consultation with, a neurologist or provider who specializes in the treatment of MS; and
  • Does not have an active Hepatitis B virus infection; and
  • Individual will not receive a live vaccine for at least four (4) weeks prior to and during treatment with Ublituximab-xiiy (Briumvi); and
  • Ublituximab-xiiy (Briumvi) is not to be used in combination with other disease modifying therapies (DMTs) for MS; and
  • Initial authorization will be for a period of up to 12 months.

Reauthorization Criteria

  • Individual has documented positive clinical response (e.g. confirmed delay or improvement of disability progression, fewer brain lesions on MRI, lower annual relapse rates); and
  • Patient is not receiving ublituximab-xiiy (Briumvi) in combination with another disease modifying therapy for MS; and
  • Reauthorization will be for a period of up to 12 months

The use of ublituximab-xiiy (Briumvi) not meeting the criteria as indicated in this policy is considered not medically necessary.

Place of Service: Outpatient

The administration of ublituximab-xiiy (Briumvi) is typically an outpatient procedure which is only eligible for coverage as an inpatient procedure in special circumstances, including, but not limited to, the presence of a co-morbid condition that would require monitoring in a more controlled environment such as the inpatient setting.



The policy position applies to all commercial lines of insured business and, if elected, ASO.
