Pemetrexed (Alimta®, Pemfexy™) is a folate analog inhibitor that disrupts folate-dependent metabolic processes essential for cell replication. Pemetrexed (Alimta, Pemfexy) is indicated for the use to treat certain cancers.

Policy Position

Pemetrexed (Alimta) may be considered medically necessary for ANY of the following indications:

Mesothelioma

  • In combination with cisplatin for the initial treatment of individuals with malignant pleural mesothelioma whose disease is unresectable or who are otherwise not candidates for curative surgery; or

Non-Squamous Non-Small Cell Lung Cancer (NSCLC)

  • In combination with pembrolizumab and platinum chemotherapy, for the initial treatment of individuals with metastatic nonsquamous NSCLC with no EGFR or ALK genomic aberrations; or
  • In combination with cisplatin for initial treatment of individuals with locally advanced or metastatic, nonsquamous NSCLC; or
  • As a single agent for the maintenance treatment of individuals with locally advanced or metastatic, nonsquamous NSCLC whose disease has not progressed after four (4) cycles of platinum-based first-line chemotherapy; or
  • As a single agent for the treatment of individuals with recurrent, metastatic nonsquamous NSCLC after prior chemotherapy; or

The use of pemetrexed (Alimta) not meeting the criteria as indicated in this policy is considered not medically necessary.

Pemetrexed (Pemfexy™) may be considered medically necessary for ANY of the following:

Mesothelioma

NOTE: In addition to the above criteria, product specific dosage and/or frequency limits may apply in accordance with the U.S. Food and Drug Administration (FDA)-approved product prescribing information, national compendia, Centers for Medicare and Medicaid Services (CMS) and other peer reviewed resources or evidence-based guidelines. Highmark may deny, in full or in part, reimbursement for utilization that does not fall within the applicable dosage and/or frequency limits.

 

Limitation of Use: Pemetrexed (Alimta) is not indicated for the treatment of individuals with squamous cell SCLC.

Place of Service: Outpatient

Pemetrexed (Alimta, Pemfexy) is typically an outpatient procedure which is only eligible for coverage as an inpatient procedure in special circumstances, including, but not limited to, the presence of a co-morbid condition that would require monitoring in a more controlled environment such as the inpatient setting.
