Carboplatin (Paraplatin®) is an anticancer drug chemotherapy drug and is classified as an alkylating agent. Alkylating agents are most active in the resting phase of the cell. These drugs are cell-cycle non-specific.

Policy Position

The use of carboplatin (Paraplatin) may be considered medically necessary when used in the treatment of ANY of the following:

Ovarian Cancer

  • As initial treatment for advanced ovarian carcinoma in established combination with other approved chemotherapeutic agents; or
  • As palliative treatment of individuals with ovarian carcinoma recurrent after prior chemotherapy, including those who have been previously treated with cisplatin.

Compendia Sources

Carboplatin (Paraplatin) may be considered medically necessary for treatment of any of the current category 1, 2A, or 2B NCCN recommendations.

Carboplatin (Paraplatin) not meeting the criteria as indicated in this policy is considered not medically necessary.

Place of Service: Outpatient

The administration of carboplatin (Paraplatin) is typically an outpatient procedure which is only eligible for coverage as an inpatient procedure in special circumstances, including, but not limited to, the presence of a co-morbid condition that would require monitoring in a more controlled environment such as the inpatient setting.
