Hematopoietic stem-cell transplantation (HSCT) refers to a procedure in which hematopoietic stem cells are infused to restore bone marrow function in cancer patients who receive bone-marrow-toxic doses of cytotoxic drugs with or without whole-body radiation therapy. Hematopoietic stem cells may be obtained from the transplant recipient (autologous HSCT) or from a donor (allogeneic HSCT) and can be harvested from bone marrow, peripheral blood, or umbilical cord blood shortly after delivery of neonates.

Policy Position Coverage is subject to the specific terms of the member's benefit plan.

Allogeneic HSCT may be considered medically necessary to treat:

  • Chronic lymphocytic leukemia; or
  • Small cell lymphocytic leukemia in patients with markers of poor-risk disease. 

Use of a myeloablative or reduced-intensity pre-transplant conditioning regimen should be individualized based on factors that include patient age, the presence of comorbidities, and disease burden.

Autologous HSCT is considered experimental/investigational, and therefore, non-covered to treat chronic lymphocytic leukemia or small lymphocytic lymphoma. Available scientific evidence does not permit conclusions concerning the effect of autologous HSCT on health outcomes for chronic lymphocytic leukemia or small lymphocytic lymphoma.

Place of Service: Inpatient/Outpatient

Experimental/Investigational (E/I) services are not covered regardless of place of service.

HSCT for chronic lymphocytic leukemia and small lymphocytic lymphoma is typically an outpatient procedure which is only eligible for coverage as an inpatient procedure in special circumstances, including, but not limited to, the presence of a co-morbid condition that would require monitoring in a more controlled environment such as the inpatient setting.


The policy position applies to all commercial lines of business


Denial Statements

Services that do not meet the criteria of this policy will be considered experimental/investigational (E/I). A network provider can bill the member for the experimental/investigational service. The provider must give advance written notice informing the member that the service has been deemed E/I. The member must be provided with an estimate of the cost and the member must agree in writing to assume financial responsibility in advance of receiving the service. The signed agreement must be maintained in the provider’s records.
