Preventive care is an important part of staying healthy. That’s why we’re making it easier for you to schedule time, once a year, with a medical professional to visit you in the comfort and privacy of your own home.
You have access to one free Highmark House Call per year, and you can schedule it a time that is convenient for you. During this 30-60 minute visit, a registered nurse practitioner will:
— Assess your current health status
— Help identify health risks
— Review any medications that you may be using
— Refer you to programs and services to help you stay healthy
— Discuss programs and services available to you that can help you stay healthy
— Provide a personal health summary — following the visit — and send a copy to your doctor so that, together, you can make informed decisions about your health
Highmark House Calls may also count toward the Highmark Wellness Rewards program.
Remember:
House Calls do not replace your annual wellness visit. These visits are designed to supplement the care you get from your primary care physician.
